Requirement for Obtaining a Surviving Member Certificate

  • A duly filled and attested application form.
  • A document providing identity (ID) proof of the applicant such as PAN Card, Passport, Voter I Card. An Aadhar Card is not compulsory, but if applicant is providing the Aadhar Card, then the name provided in the application should match the name provided in his/her Aadhar Card.
  • If not minor, then a self-declaration in the prescribed format.
  • The photograph of the applicant is required to be matched with his/her photograph on the Aadhar Card. The photograph of the applicant will be captured through a web camera during application submission or verification.
  • Documents specifying applicant’s present residential proof such as electricity bill, telephone bill, or water bill.
  • Death certificate of the deceased.
  • Area SDM can be approached or concerned O/o the Deputy Commissioner between 10:00 AM to 1:00 PM on any working days.
  • It will take approximately 14 days to issue the certificate.
  • All documents are required to be attested by a Gazetted Officer.
